Root Cause Analysis System

Structured Problem Cause and Corrective Action Analyzer

A Project ARGOS Application

Root Cause Analysis System is a web-based professional investigation and decision-support application for defining problems, examining evidence, exploring and verifying causes, developing corrective actions, confirming implementation and effectiveness, reviewing traceability, and assessing closure readiness.

Many investigations lose coherence because the problem statement, evidence, suspected causes, corrective actions, and verification results are kept in separate files or treated as independent tasks. Root Cause Analysis System brings the full investigation chain into one structured record so users can see how each conclusion is supported and where additional review is needed.

The application supports professional judgment. It does not declare a cause proven, approve an action, or close an investigation on behalf of the responsible organization.

A Complete RCA Working Environment

The application organizes the investigation through five connected workspaces and a professional report output.

  1. PROBLEM: Define the observed gap, context, impact, evidence, immediate response, containment, and correction.
  2. CAUSES: Explore candidate causes with multiple analytical methods and verify causal propositions against evidence.
  3. ACTIONS: Develop cause-linked corrective actions, assign ownership and timing, assess coverage, and record implementation.
  4. VERIFY: Plan and complete implementation and effectiveness reviews, including recurrence and follow-up.
  5. ANALYZE: Review the complete investigation, traceability, analytical findings, and closure readiness.

Preview, print, or save the structured investigation report and preserve an editable external investigation file.

Define the Problem and Preserve the Evidence

Create a named investigation with an internal reference, then record the problem statement, expected and actual conditions, scope, magnitude, impact, location, timeframe, affected parties, severity, and supporting evidence. Immediate response, containment, and correction remain distinct from long-term corrective action.

The Problem Definition Assessment identifies missing or weak structural elements before the investigation moves into cause analysis.

Explore Causes with the Right Method

CAUSES provides several complementary methods. Users select the approach that fits the problem and available evidence; the application does not force every method into every investigation.

  • 5 Whys with branching paths and automatic candidate-cause creation
  • Fishbone or Ishikawa analysis with editable and custom categories
  • Pareto Analysis for frequency or impact prioritization
  • Cause Map for connected causal chains and interacting conditions
  • Barrier and detection review where preventive or detection controls require examination

Candidate Cause Inventory with origin and verification status

Verify Causes Against Evidence

Exploration produces candidate causes, not automatic conclusions. Evidence-Based Cause Verification records the verification question, evidence reviewed, expected relationship, observed result, conclusion, and rationale. Each candidate can be classified as Supported, Not Supported, Inconclusive, or Not Investigated.

This separation helps prevent a plausible explanation from being presented as a verified cause without adequate evidence.

Develop Traceable Corrective Actions

ACTIONS connects corrective work to the supported causes it is intended to address. Record the action description, type, priority, linked cause, responsible owner, target date, status, implementation details, and evidence reference.

Cause Coverage shows whether each supported cause has at least one retained corrective action. Action Assessment identifies structural gaps such as missing ownership, missing dates, incomplete implementation information, or cause-linkage concerns.

Confirm Implementation and Effectiveness

VERIFY distinguishes implementation from effectiveness. An action can be installed, issued, trained, or released without yet demonstrating the intended result.

Implementation Verification confirms that the approved action was put in place as intended.

Effectiveness Verification evaluates results against defined criteria, baseline conditions, evidence, and an appropriate observation period.

Verification Assessment checks completeness and internal consistency and can identify contradictory conclusions or missing follow-up.

Analyze the Complete Investigation

ANALYZE consolidates the investigation and organizes the review through four sections:

Investigation Overview summarizes the recorded elements and key counts.

Traceability Analysis follows supported causes through corrective action, implementation, and verification.

Analytical Findings identifies conditions that require attention or professional review.

Closure Readiness indicates whether configured structural findings remain unresolved.

Closure readiness is advisory. Formal closure remains subject to professional approval and the organization’s legal, safety, regulatory, quality, and governance requirements.

Professional Reports and Editable Investigation Files

Preview the complete investigation report before printing. Use the browser print dialog for direct printing or PDF saving. Reports include the investigation record, cause-analysis results, corrective actions, verification cycles, traceability, analytical findings, and closure-readiness information.

Save Investigation File creates an external editable investigation record in JSON format. It can be reopened in the application or transferred to another authorized device. This file is separate from the formatted report.

Integrated Methodology Terminology and Guidance

The application includes an integrated RCA methodology and terminology reference, plus plain-language tooltips for fields, controls, methods, and analytical terms. Guidance remains available within the working environment while the investigation is being built and reviewed.

English, Spanish, and Italian

The interface is available in English, Spanish, and Italian. The selected language applies across navigation, labels, controls, tooltips, methodology and terminology, diagnostics, and system-generated report text. User-entered investigation content remains exactly as entered.

Designed for Professional Investigation Work

Root Cause Analysis System can support quality, operations, manufacturing, supply chain, engineering, maintenance, safety, customer service, process improvement, consulting, audit preparation, project work, and other environments where a documented investigation must connect evidence, causes, action, and results.

It can be used by managers, process owners, quality and operational professionals, analysts, consultants, educators, and students who require a structured RCA environment rather than a static worksheet.

Web Based Access

Root Cause Analysis System runs in a modern web browser. No desktop software installation is required. After purchase, the customer receives the access instructions and unique license information needed to activate the application.

Important Professional Use Notice

The reliability of the analysis depends on the completeness, accuracy, relevance, and integrity of the information entered by the user. The application does not test physical evidence, determine technical truth, replace subject-matter expertise, or provide legal, regulatory, engineering, safety, medical, or quality-system approval.
